|  Applying to the Metro Omaha Wolves Socctoberfest
Please read this document completely before clicking to the application form. It will save you some work and frustration.
COMPLETING THE ON-LINE APPLICATION FORM STEP 1: Complete the application. ALL REQUIRED FIELDS MUST BE FILLED IN or the application will not be accepted. BE SURE TO SUPPLY AN EMAIL ADDRESS THAT IS RELIABLE AND ACCURATE as this is the ONLY way we will contact you if your team is accepted. STEP 2: Once you hit Submit, your application will be sent to the tournament. THE FORM WILL NOT BE ACCEPTED if you do not supply an email address or fail to check the verification boxes at the very end of the form. STEP 3: When you submit your application, you will be given a web page that contains your Team ID Number. You should make two copies, one to keep on file and one to mail with your fees. YOU WILL NEED YOUR TEAM ID NUMBER TO CHECK YOUR STATUS, CONFIRM IF ACCEPTED, MODIFY YOUR APPLICATION... pretty much anything that has to do with the tournament. As this is also your private log-in code, you should be careful about who has access to it. STEP 4: Check your email. You should have a completed application certificate for each team in case you couldn't print the page on Step 3. PAYMENT: Pay by credit card when you register, or mail a check to the address supplied in your application certificate, or send an online payment to our PayPal account (at Treasurer@MetroWolvesSoccer.com). You are not required to have your own PayPal account to use a credit card via PayPal. Please include your Club, Team Name, and Team ID number when making your payment. If paying by check, please plan for adequate mailing time when completing your registration. By applying for this tournament, you give Phoenix Futbol Club, doing business as Metro Omaha Wolves Soccer Club, permission to contact you regarding activities surrounding this tournament and other activities. CHECK DEPOSITS: All checks received will be deposited when your team is accepted, or as soon thereafter as checks are received. Online payment does not necessarily mean that your team is accepted. If your team is NOT accepted or we do not have a tournament bracket appropriate for your age / gender, you will receive a FULL refund or your check will be shredded. Clubs registering more than one team per check submitted will be issued partial refunds if not all teams are accepted. COACHING CONFLICTS Please note in the comments on your application if you are coaching more than one team in the tournament. The tournament committee shall make every effort to avoid conflicts and/or put your games near each other for logistical convenience if you are coaching two teams but we are not always successful in avoiding 100% of such conflicts. If you are coaching more than two teams, then no guarantees can be made that there won't be some conflicts - be prepared to have an assistant coach take over for some games. Please make sure the coach's name is spelled the same way on both team applications - conflicts may not be flagged if a name is misspelled or the coach is Tom on one team and Thomas on another, for example.If you have significant time constraints that would require special scheduling for the tournament, this may not be the best tournament for you to apply to. As our tournament as grown so has the complexity of scheduling, and we are not able to accommodate much in the way of requests beyond the multiple team conflicts. Thank you for your understanding. As a condition of renting the CHI Health MultiSport Complex, SocctoberFest is a Stay to Play tournament using their approved housing partner (CSTT). Teams traveling from outside the greater Omaha area are expected to stay at MultiSport Complex affiliated hotels or risk not being accepted to the tournament. To book your hotel, please go to this link. Division AgesPlease use the table below to verify your age group BEFORE proceeding to the registration form. Your application will not be accepted by the on-line registration system if the DOB of the oldest player is not consistent with the selected age group.
